Yudi Polatoff LNHA
“I’m a people person”. That’s the first thing Yudi usually says when describing himself. And it very apparent when you meet him that it’s a skill he puts to use all the time. Yudi gets to know his residents personally, and looks forward to taking a break from his administrative chores to walk the halls and “chat it up” with them. His leadership skills have helped to create a team that follows in those same footsteps, and that has helped to transform the Ontario Center from a municipal facility to a home. “We’re our own little neighborhood here now, not a county facility but a private residence. The folks here feel that and so do their families. And we plan to keep on building on that.” If anyone can do it, It’s Yudi.
Siobhan Baker RN
Director of Nursing
Every resident at Ontario Center – and most our staff, too – are beneficiaries of a Siobhan’s sincerity and depth of knowledge. There simply aren’t any questions relating to the care and wellbeing of our residents she doesn’t have a ready answer for. Her expertise and unwavering dedication to excellence is paramount, and she leads an outstanding with confidence and compassion. Sounds like a good leader, right? But Siobhan is so much more. She’s quick to volunteer her time and energy for activities here and in our community. She’s a willing shoulder to lean on for resident and staff alike, and a downright great listener. But ask her why she chose elder care, and you’ll hear the real reason that we wanted her on our team. “I love elder care. It’s in my heart. And I am so blessed to be able to lend my talents to the people at Ontario Center.” And we’re blessed to have her!
Lindsay Huson OTR/L
Director of Therapy
“Let’s do this”. That ‘s Lindsay’s motto. And that’s her philosophy when it comes to running a therapy department. Whether it’s making sure her staff has the equipment they need, or encouraging a resident to tackle another day of physical therapy, her can do attitude and team approach keeps everyone positive. And when Lindsay says “Let’s do this” to a resident, she means US. She’s a hands-on therapist who gets her greatest sense of fulfillment from working one-on-one with her residents, helping them make the little strides and the big ones, and keeping them focused on the goal. “I love being a therapist. It’s the greatest joy to see someone come here unable to get around on their own, and after a few weeks or a few months, seeing them walking down the hall and knowing you helped them get there. It’s my dream job and I’m living it every day right here at Ontario Center”.
Lindsay’s entire team here is dedicated to helping each and every patient they encounter achieve the highest possible level of function and independences. And they don’t take “No” for an answer.
Dawn Clark LMSW
Director of Social Services
Dawn is a bit of a juggler, and she says that’s one of her greatest assets. “I have two boys at home doing studying and chores, playing ice hockey, soccer, lacrosse, they’re on school teams and travel teams, and I have a very active dog. So keeping a lot of things going all at one is what I do every day. And it’s no different here. Settling folks into a new home and getting them acclimated and introduced around, or helping a rehab resident make all the necessary preparations for returning home means balancing an awful lot of things at once. So it’s not unusual to have a lot of things going on all at the same time, and I love that. I’m good at it. And it keeps every day fresh.” No wonder things run so smoothly here.

Our team also works hard to keep therapies as consistent as possible, tracking all communications through an advanced electronic medical record system. This ensures immediate transmission of information across the entire care network.
